A) I consent Perfection Cosmetic staff to perform a test patch on me prior to my treatment, I have been fully informed of the contra-indication and accept all possible outcomes. I have answered all questions fully. I hereby certify that I have been fully informed of the nature of the procedure, expected outcome and possible complications. I understand that there can be no guarantee or assurance as to the final result that maybe obtained.
B) Perfection Cosmetic advises you to follow the aftercare advice for the 48 hours as follows:
C) Protect your skin with an SPF and stay out of the sun, Avoid sunbeds and self-tanning products, Don’t apply any perfumed products to the treated areas including shower gel, deodorant and body spray, Opt for lukewarm/warm showers rather than hot showers or baths, Avoid swimming, saunas or steam rooms, Keep skin bare or wear loose fitting clothing to minimise any irritation, Moisturise with a perfume-free product, Aloe-vera gel may help to cool your skin and minimise any redness.
D) If you are unable to attend your appointment(s) we require 24 hours notice in advance of your appointment time in order to reallocate your appointment to another client or 50% of the fee is charged for any single appointment. If you have purchased a course of treatments and fail to attend the appointment, 1 treatment will be forfeiting from the course. Perfection Cosmetic will inform you of any preparation needed prior. Failure to follow this will result in cancellation of your appointment or additional fees charged.
E) Payment for a course of treatments is due in full prior to the treatment(s) and all treatments must be taken within 12 months of the date of purchase. Any unclaimed treatment after 12 months will be forfeited. A refund is not made if you stop attending at any stage through the course of treatment.
F) Late arrivals may result in reduced treatment time or forfeiting of the appointment. No promotion can be used in conjunction with any other offer.
E) All offers and promotions must be paid in full before the treatment. They are non-refundable and non-transferable, however if you are unable to complete the course due to medical reason where evidence from the doctor is required, Perfection Cosmetic will deduct the full price of all taken treatments plus those forfeited for missed appointments and refund the difference.
G) In the event of short/long term side effects such as bruising, reddening, burning, swelling, blistering, hypopigmentation, hyperpigmentation or any scarring or permanent discoloration you need to let Perfection Cosmetic know within 3 working days and no refund will be issued.
H) Perfection Cosmetic will not take any liability of any short/long term side effects such as bruising, reddening, burning, swelling, blistering, hypopigmentation, hyperpigmentation or any scarring or permanent discoloration and you will be consenting to the treatment(s) and accept all possible side effects.
I) For any single treatments the fee must be paid in full prior to treatment. For multiple booking for single treatments 50% deposit is required. Please note prices may vary depending on the volume of area to be treated and the treatment type. Prices stated are the starting price and may vary for each individual.
J) Perfection Cosmetic will assess your suitability prior to any treatment, if you are unsuitable for the treatment you will be advised by the practitioner. In these circumstances you will only be liable for the cost of the initial consultation, if applicable.
K) Perfection Cosmetic will not be liable in contract, or otherwise for any economic loss (including without limitation, loss of profit). Or for any other special, indirect or consequential loss or damage rising out of or in connection with its provision of any goods and/or services to the client.
L) It is the clients’ responsibility to ensure and provide Perfection Cosmetic with all the up to date relevant medical details prior to each treatment. Perfection Cosmetic will not be liable for any damage which occurs as a result of the client’s failure to disclose any relevant medical details.
M) The client agrees to comply with all recommendations given to them by, or on behalf of Perfection Cosmetic regarding the aftercare of the treated area and will not be liable for any damage caused by the client’s failure to follow any of recommendations regarding the care of the treated area.
N) Perfection Cosmetic endeavours to treat all its clients fairly. If, however, you have an issue with any matter in relation to your treatment you are entitled to lodge a complaint, by following our complaint procedure found on this website.
O) When a complaint is received Perfection Cosmetic may require you to attend an additional consultation with the company specialist, if this is deemed appropriate. If you are not satisfied with this initial attempt at resolution, or have any objection to being seen by this practitioner Perfection Cosmetic will try to offer an alternative. Nothing in these terms of business shall exclude or limit Perfection Cosmetic’s liability for any personal injury resulting from Perfection Cosmetic’s negligence.
