However, if for any reason, you are dissatisfied with the service you have received or have a complaint about your experience with us, we ask that you follow this procedure, so that we can address your concern at the earliest opportunity.
The quickest and most effective way of resolving any issue is to bring the matter to the attention of the clinical team. They will listen carefully to your concerns and do what they can to resolve the matter for you.
If however you feel your complaint has not been resolved, or you do not feel you are able to raise the complaint in person, you can write to us at our registered address 112,116 Chorley New Road, Bolton, BL1 4DH.
Please provide as much detail as possible, including full details of your complaint, and the date and time of the relevant appointment or visit. We will acknowledge your written complaint within 7 to 14 working days. We will conduct a thorough investigation and respond back to you with a full and final response within 8 to 12 weeks. If we need more information as part of the investigation we will contact you.
We will ensure that all complaints are investigated, documented and where appropriate, action taken to prevent a recurrence, if we need more time to investigate we will contact you to request the relevant information.
